Consent Forms

Once you have edited the form, save the document as a PDF. Delete any wording that does not apply and be sure to erase or delete parentheticals that are in the forms for instructional purposes. To do so, while you have the form open in Microsoft Word, click the circular Windows button at the top left and choose Save As. In the dialog box that appears, select the “Save as type” PDF value and click Save. You will need to attach this PDF to the protocol in the WVU+kc system.

Parental Consents

Please note that if: 1) The researcher is meeting the parent or guardian and the child at the same time to discuss participation, and 2) The child is under 12 years of age, then the investigator must make every effort to explain the study to the child and the child must agree. Furthermore, if you would like to combine the consent and assent documents in the case that both of the aforementioned requisites are met, you should add a new signature line for the parent following the added statement “My child appears to understand the research to the best of his or her ability and had agreed to participate.”
